The need for unified guidelines on ONS, EN and PN
Guidelines and consensus reports in the management of diseases guide and help clinicians practice evidence-based, updated management. In medical nutrition therapy, after or concomitant with dietary interventions, oral, enteral, and/or parenteral nutritional supplementations are required. A short report, is clear, and has clear-cut recommendations that will guide health care professionals in indications for certain patients, choice, practical application, and follow-up oral nutritional supplements (ONS), enteral nutrition (EN) and parenteral nutrition (PN) would facilitate the application and success of medical nutrition therapy.
In this regard, the Turkish Society of Clinical Enteral and Parenteral Nutrition (KEPAN) took the liberty of studying and practically guiding three reports on the use of ONS, EN, and PN.
For each consensus report, a working group was created composed of academicians belonging to a variety of specialties, who are experienced and involved in the application of clinical nutrition. The working group members include physicians, dietitians, nurses and pharmacists, with representation of a variety of different departments in medical nutrition. Each consensus work had a study leader.
The ONS consensus report on ONS use in medical nutrition therapy was developed by a study group (8 working group academicians and 19 expert group academicians) under the organization of KEPAN. The ONS consensus report was generated in 5 online and face-to-face phases from June 2019 through June 2021. At the end (Delphi rounds), this report provides 22 clear-cut recommendations in a question-answer format. It was published both in Turkish (1) and in English (2).
The EN consensus report on EN use in medical nutrition therapy was developed by a study group (12 working group academicians and 17 expert group academicians) under the organization of KEPAN. The EN consensus report was generated in 5 online and face-to-face phases from December 2019 through October 2022. At the end (Delphi rounds), this report provides 24 clear-cut recommendations in a question-answer format. It was published both in Turkish (3) and in English (4).
The PN consensus report on PN use in medical nutrition therapy was developed by a study group (9 working group academicians and 10 expert group academicians) under the organization of KEPAN. The EN consensus report was generated in 5 online and face-to-face phases from November 2019 through June 2021. At the end (Delphi rounds), this report provides 22 clear-cut recommendations in a question-answer format. It was published only in Turkish (5) and publication of the report in English is in process.
Conclusion and comment from KEPAN
We believe that these consensus reports could have a significant effect on the optimum use of ONS, EN and PN in the context of medical nutrition therapy when clinicians manage everyday patients.
